As a nurse with Kore Cares, you will assist the client with nursing tasks, as outlined in the care plan, to allow the client to remain in their home.
Qualifications:
2 years of nursing experience, preferred
Must have active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of SD
Must be able to physically perform the duties of the position
Must be able to read, write, understand and speak English
Must have reliable transportation and current valid driver’s license
Proof of automobile insurance and eligibility for employment in the United States
Duties include but are not limited to:
Nursing tasks may include but are not limited to: wound dressing, medication set up, monitor medication compliance, assess vital signs, assess skin condition, assess nutritional status, assess mobility, assess usage of glucometer, toe/nail care, catheter care, assess health, provide teaching/health education, communicate with physician and pharmacist, blood draws
Assesses the full scope of patient needs using the nursing process on a continuous basis
Observes signs and symptoms and reports changes in the patient’s condition to the physician and/or other appropriate health care professionals
Provides teaching to client and client family members
Uses knowledge and judgement needed to meet nursing needs of the patient
Responsibilities:

Easily adapt to change
Treat all co-workers, clients, and others with dignity and respect while on duty
Provide care to client in accordance with care plan
Provide care for client that maintains client dignity and meets physical, emotional and spiritual needs
Immediately report any changes in client condition, accidents or incidents to the Agency office.
Thoroughly document care provided in ClearCare
Comply with agency handbook, policies and procedures
