Horizon Health Care is seeking a full time Patient Support Navigator. This is a grant funded position. The Patient Support Navigator will educate and assist patients and community members on their potential eligibility for insurance, and how to enroll in assistance and insurance programs. Successful candidates for Patient Support Navigator should be passionate about providing high-quality, patient centered care.
Horizon Health Care offers a team-oriented work environment focused on meeting the needs of individual patients while supporting and contributing to the health and wellness of our communities. Ideal candidate should have an Associate Degree or Bachelor’s Degree in Business, Medical, Communications, Finance/Accounting and/or related field is required. If you are a positive team player and enjoy working in a busy environment, this position is for you.
*This position will be located in the Huron or Aberdeen clinics
